Camper Health History
According to the New York State Health Department we are required to collect a Camper Health History for all participants. This form can be filled out online when you register your child for camp or at any other time using your ultra-camp account. If you miss any sections of this form you will be required to provide this information at drop off. If you cannot fill out the form online it can be found on this site, the completed form can be dropped off with your camper or emailed to owahta@cornell.edu.
Immunization History
We are required to collect immunization dates for; diphtheria, haemophilus influenza type B, hepatitis B, measles, mumps, poliomyelitis, rubella, tetanus and varicella (chicken pox). This information can be filled in online through ultra-camp, or you can send an immunization record from your doctor to owahta@cornell.edu.
Medication
Any medications (Over the counter, prescription, topical ointments or medicated lotions, vitamins, sleep aids, etc) must be in their original packaging and accompanied by a licensed authorized providers orders. These orders must include times for administration. Any medications that are not accompanied by orders will not be administered.
